If you purchased a product online from the Company (not directly from your physician’s office) and received it damaged, all claims for products damaged during shipment must be received within 72 hours of delivery of your product(s). Please inspect your shipment immediately upon receipt so that you do not miss this time window if your product is damaged.
Any claims for damaged product(s) must be submitted to email@example.com within this timeframe. You will need to provide detailed information on any product damaged, including photo proof. You will be responsible for return shipping, and once we have received your item, you will be refunded the full purchase price minus shipping costs onto your original method of payment.
All sales are final. No returns or credits will be issued, with the exception of items damaged during shipping.
